Welcome to Drakes Creek Middle School!

The following list contains items a new student needs to register for school, as required by STATE LAW, for all school age children.  These items are necessary before a student can start classes.

KENTUCKY IMMUNIZATION CERTIFICATE

CURRENT PHYSICAL EXAM on the KY Physical form (WITHIN THE PAST YEAR)   

If you need any other special Ky Health forms (such as a KY immunization certificate or exemption) you can visit The Kentucky Department of Education Health Website.

PROOF OF RESIDENCE

GUARDIANSHIP PAPERS (if divorced or other special circumstances)

We also need a photocopy of your child’s SOCIAL SECURITY NUMBER to be able to place your child in the computer system.

You will also need to print and fill out the Registration Form, Home Language Survey, and Enrollment sheet below.  Please bring these when you come to register.
